New Patients

Please arrive 30 minutes early for your first visit or complete the New Patient Registration Form for Adults or New Patient Registration Form for Children prior to your appointment. Bring your insurance card and all medication (prescription and non-prescription) that you are taking. For Pediatric patients, please bring a copy of your child’s immunization record.

Established Patients

At each visit, please notify us of any changes in your address, phone number or insurance information. Bring your insurance card to each visit. Please bring all of your medications (prescription and non-prescription medications). For Pediatric patients, please bring a copy of your child’s immunization record to every well check visit.

If you are running late, please call our office. If you will be more than 15 minutes late, you may be asked to wait or to reschedule your appointment. If you must cancel your appointment, please try to notify us 24 hours in advance to avoid a possible $25 late cancellation fee. We wish to be respectful of your time as well as that of other patients.

When the Clinic is Closed

If you have an emergency after office hours, please go to the nearest emergency room or call 911. Our doctors have hospital privileges at several Cedar Park, Round Rock and Austin hospitals. If your problem is not an emergency, please leave a message for the appropriate staff person. Messages will be received on the next business day. Please do not page the provider on call for non-emergencies such as appointment scheduling, refilling prescriptions, getting test results or referrals, or for billing and insurance questions. Please call the office during office hours for these matters.

Referrals

Many insurance plans require a referral/authorization from your primary care physician before you schedule an appointment with a specialist. Please call our office when a referral is needed. An appointment with your doctor may be necessary and authorizations may take 5 days to process through the insurance.

Medical Records

If you are transferring from a previous doctor, you may request a copy of your medical records (in most cases, a limited copy, which includes the most recent and important information, will be adequate) to be sent from the previous office by completing a Release of Information Form.

Prescription Refills

If you have any questions concerning your medications, please call us. If you need a medication refill, please call your pharmacy. There may be a 3-day turnaround time for refills. Refill requests received after noon on Friday may not be filled until Monday. It is best to avoid running out of your regular medications to avoid any delays. Refill requests for ADHD medications require a call to our office as there may be particular questions we will need to ask in order to refill them.

Payment for Services

All co-pays are due at the time of your appointment. All other payments are due at the time of service, unless other arrangements have been made. If we bill your insurance, you may still be responsible for any services not covered by your insurance.
